Please include your full name and state that you have read and agree to the statement on our web-site, giving Dino Jump permission to post your childs(ren's) picture on our web-site. We may also choose to use your childs(ren's) picture in our new advertisements, no names will be included or used in any form of public advertisements we choose to use the photographs provided.
